Extending Standards of Care
REACH helps automate the standards of care and clinical care pathways advocated by the facility (hub), thereby enabling the hub to bring its high standards of care to its network of hospitals that may include rural, underserved, and urban facilities. This automated standard of care is made available to the REACH network facilities as a clinical consultation module, enabling care to be consistently applied reducing volatility and more predictability, which are key quality metrics as well. Because the consultation module is customized for the hub and its participating spokes (even containing the hub’s logo), REACH essentially enables the hub to “brand” and “market” its standards of care to spoke facilities. Furthermore, by making the consultation modules available through REACH’s web portal, REACH enables universal access to these services. To support patient safety and help prevent medication errors, the consultation module also captures/displays critical information for prescribing or recommending treatment including vitals (e.g. using patient weight to automatically calculate the correct dosage for tPA), labs, and contraindications.
Data Capture &
Reporting
Seamless data capture and integration is a hallmark of the REACH solution. REACH automatically captures consultation data and stores in a central location for secure, authenticated access by the hub or relevant spokes. All data is time-stamped and thorough, making it invaluable for record-keeping, audits, litigation documentation, and Joint Commission certification. In addition, one customer’s compliance officer indicated that the captured data supports the highest level of billing (Level 5). REACH also includes an on-demand reporting package which enables customer to benchmark their network’s performance and improve quality metrics (e.g. # consults, tPA administration, # transfers, etc.).
